Application for Community Preservation Act Funding
Application for a Dog License - Required for all dogs, 6 months or older.
Application for a Permit to Operate a Taxi in
West Tisbury - Print and fill out both pages of this Application. Mail or bring
it with a check for $15 made out to the "Town of West Tisbury" and 2 passport-sized photos to the Police Department at P.O. Box 520, West Tisbury.
You must
have a valid Massachusetts Driver's License and prospective employment with a
West Tisbury licensed taxi company. Taxi Permits must be picked up at the
station.
